STEP 1: Application & Consultation

To start your process you will need to fill out a "Client Registration Package" which can be downloaded at the bottom of this page. Return your completed package to the office:

STEP 2: Initial Assessment of Client

This comprehensive assessment allows our healthcare providers to gather essential information about the client’s health status, daily living needs, and personal goals. It forms the basis for developing a tailored care plan that addresses the client’s unique needs.

STEP 3: Customized Care Plan

Based on the initial assessment, we’ll develop a personalized care plan tailored to your health needs and lifestyle.

STEP 4: Insurance Authorization (If Applicable)

At AILHH, we understand that each client has unique needs and preferences. That's why we offer flexible care plans that can be customized to meet the specific needs of each client.

STEP 5: Finalize Service Agreement

At AILHH, we ensure that every client is provided the level of care and desired services at the agreed upon pricing indicated at time of enrollment.

STEP 6: Payment of Deposit or Copay (If Applicable)

A deposit payment or copay must be made prior to services starting.

STEP 7: Meet Your Care Team

We’ll match you with the best healthcare professionals to provide the care you need, ensuring you feel comfortable and supported.

STEP 8: Start Receiving Care

Begin receiving top-quality care in the comfort of your home, with ongoing support and communication from our team.
